PGY2 Infectious Diseases Application Information

Application Requirements

In order to be considered for this residency, the applicant must:

  1. Possess a strong interest in infectious diseases clinical pharmacy practice
  2. Be a U.S. citizen or permanent resident
  3. Complete an ACPE-accredited PharmD Degree program
  4. Be enrolled in or complete an ASHP-accredited PGY1 Pharmacy Residency
  5. Participate in the ASHP Resident Matching Program
  6. Be eligible for pharmacist licensure in the state of Texas

Once accepted into the residency program, candidates should submit a completed Job Application by May 1 and obtain a Registered Pharmacist License in the state of Texas by August 1.

The applicant must submit the following application materials via the Pharmacy Online Residency Centralized Application Service (PhORCAS) at https://portal.phorcas.org/.

  1. Letter of interest/intent
  2. Current curriculum vitae
  3. Three professional letters of recommendation
  4. Official college of pharmacy transcripts
