Community Education and Resources

We love our community as well as our patients. A healthy community depends on both education and resources, so we aim to provide both to organizations to help the people of Bexar County stay active, happy and healthy.

Community Involvement

Our outreach representatives assist organizations, schools and churches with providing community-wide training and support to families who need health-related services. Whether that’s completing enrollment applications, hosting educational workshops or attending health fairs, we’re here to help.

We provide educational workshops to help families learn about CareLink and our various grant services for women's health, colorectal screening and more. We also help patients submit applications for the new University Community Care Plan (UCCP). This new Marketplace plan offers additional coverage, and our team is ready to provide you with information.

Connecting Kids to Coverage

Our Connecting Kids to Coverage team helps parents enroll their children into Medicaid or CHIP. We also offer application assistance for pregnant individuals who need help with the Medicaid for Pregnant Women or CHIP Perinatal application. Our team offers one-on-one assistance throughout the application process, which usually takes 5 to 7 business days.

Medicaid and the Children’s Health Insurance Program (CHIP) both provide free or low-cost health insurance for children and teenagers up to age of 19. Enrolled children receive regular check-ups, hospital care, dental care, mental health services, immunizations, prescriptions and more.

HEALTHY FAMILIES BEXAR

Healthy Families Bexar is an early intervention and home-visit program that empowers parents, taps into their existing skills and brings out their best. Using the evidence-based model provided by Healthy Families America, Healthy Families Bexar offers parent education and support to enhance outcomes for young children and families.

We encourage pregnant moms and families with infants up to 3 months old to apply for program participation. Once enrolled, we connect families with caring and well-trained navigators who conduct home visits tailored to meet each family’s specific needs.

Parent Café

Parent Café is a structured, small-group conversation that focuses on transformation and healing within families. Cafés provide a physically and emotionally safe space where people talk and listen to each other about topics that matter to them.

Parents and caretakers create strategies from their own wisdom and experiences to strengthen their families. Families with children up to 5 years of age are encouraged to participate.

Get FIT

Regular screenings can prevent colorectal cancer, which is the second leading cause of cancer deaths in men and women combined in the United States. One screening method is the fecal immunochemical test (FIT), an annual test that detects blood in the stool. Our program aims to prevent the development of colorectal cancer and increase awareness by providing free FIT screenings to eligible CareLink members in Bexar County and Maverick Financial Assistance Program members in Maverick County ages 45-75.

FIT screenings must be ordered by a primary care physician and are available at no cost to participants of the Get FIT program. To accommodate our members, our patient navigators offer English and Spanish community education sessions virtually and in-person. Once members complete the class, they are eligible for a $25 credit toward their account balance. Get FIT navigators provide personalized assistance to support and guide members through the screening process, as well as provide additional treatment information if necessary.

MUJERES CON CONFIANZA

Mujeres Con Confianza (MCC) is a CareLink grant program dedicated to educating providers, staff and patients on endometriosis, polycystic ovary syndrome (PCOS) and uterine fibroids.

The program aims to improve early diagnosis and treatment rates for active CareLink members who are women between the ages of 18 and 45. The program consists of three educational sessions, which are offered in-person and online in both English and Spanish.

Participation in the program is free, and CareLink members may also receive a participation incentive and earn credit toward their member account balances.

Education sessions:

  • Session I: Polycystic ovary syndrome (PCOS)
  • Session II: Endometriosis
  • Session III: Uterine fibroids
