Please have your child’s pcp/clinic and insurance information ready (if applicable) before starting registration form. Most insurances are accepted. Financial assistance available to those who qualify.

Please provide an email address to receive an appointment email confirmation. You can also print or save a screen shot of the confirmation page. When scheduling an appointment, do not use special characters such as ñ, á, é, í, ó, ú.
